Estos trastornos se producen cuando el cuerpo no descompone (digiera) adecuadamente las mol\u00e9culas complejas de az\u00facar, tambi\u00e9n conocidas como mucopolisac\u00e1ridos o <a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK544295\/#:~:text=Introduction,vast%20amount%20of%20biochemical%20processes.\">glicosaminoglicanos<\/a> (GAG).&nbsp;<\/p>\n\n\n\n<p>Normalmente, los GAG contribuyen a la formaci\u00f3n de huesos, tendones, cart\u00edlagos, c\u00f3rneas, piel y tejido conectivo. Tambi\u00e9n se encuentran en el l\u00edquido que lubrica las articulaciones. Estas complejas mol\u00e9culas de az\u00facar se reciclan y descomponen dentro de las c\u00e9lulas mediante un proceso de 11 pasos; cada paso cuenta con una enzima especializada que ayuda a descomponer estas complejas mol\u00e9culas de az\u00facar.&nbsp;<\/p>\n\n\n\n<p>Sin embargo, en la MPS, una de las enzimas falta o funciona mal, lo que impide la descomposici\u00f3n de los GAG. Nos centraremos \u00fanicamente en la MPS I y la MPS II.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Mucopolisacaridosis tipo I (MPS I)<\/strong><\/li>\n<\/ul>\n\n\n\n<p>La mucopolisacaridosis tipo I, o MPS I, es un trastorno multisist\u00e9mico, lo que significa que esta forma afecta a muchas partes del cuerpo. Sin embargo, los dos tipos de MPS difieren en el gen afectado. La MPS I se caracteriza por una mutaci\u00f3n en el gen IDUA, que produce la enzima alfa-L-iduronidasa (IDUA). En cambio, en la MPS II, se produce una mutaci\u00f3n en el gen IDS, que produce la enzima iduronato 2-sulfatasa (I\u2082S).<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Patr\u00f3n de herencia<\/strong><\/li>\n<\/ul>\n\n\n\n<p>El patr\u00f3n de herencia es otra diferencia significativa entre MPS I y MPS II.&nbsp;<\/p>\n\n\n\n<p>La MPS I se hereda con un patr\u00f3n autos\u00f3mico recesivo, lo que significa que ambos padres deben transmitir el gen IDUA defectuoso o mutado para que el ni\u00f1o se vea afectado.<\/p>\n\n\n\n<p>Por otro lado, la MPS II se hereda con un patr\u00f3n ligado al cromosoma X, lo que significa que el gen IDS mutado est\u00e1 presente en el cromosoma X. Este patr\u00f3n afecta principalmente a los varones porque s\u00f3lo tienen un cromosoma X, aunque las mujeres pueden ser portadoras y pueden mostrar s\u00edntomas leves en casos raros.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Prevalencia: Poblaciones afectadas<\/strong><\/h2>\n\n\n\n<p>La forma grave de MPS I (s\u00edndrome de Hurler) afecta a alrededor de <a href=\"https:\/\/rarediseases.org\/rare-diseases\/mucopolysaccharidosis-type-i\/\">1 de cada 100.000 reci\u00e9n nacidos<\/a>, mientras que la forma menos grave es rara y afecta <a href=\"https:\/\/rarediseases.org\/rare-diseases\/mucopolysaccharidosis-type-i\/\">aproximadamente 1 de cada 500.000 reci\u00e9n nacidos<\/a>.&nbsp;<\/p>\n\n\n\n<p>La MPS II o s\u00edndrome de Hunter es poco com\u00fan y solo afecta a alrededor de <a href=\"https:\/\/rarediseases.org\/rare-diseases\/mucopolysaccharidosis-type-ii-2\/\">1 de cada 100.000 a 170.000 ni\u00f1os varones<\/a>.&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Diagn\u00f3stico y pruebas para MPS I y MPS II<\/strong><\/h2>\n\n\n\n<p>Los profesionales de la salud realizan procesos o pruebas diagn\u00f3sticas para confirmar el diagn\u00f3stico de MPS I y II. Sin embargo, la terapia de reemplazo enzim\u00e1tico y el trasplante de c\u00e9lulas madre hematopoy\u00e9ticas se utilizan como terapias est\u00e1ndar para ralentizar la progresi\u00f3n de la enfermedad y mejorar la calidad de vida.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Terapia de reemplazo enzim\u00e1tico (TRE)<\/strong><\/li>\n<\/ul>\n\n\n\n<p>En la TRE, se administra una forma gen\u00e9ticamente modificada (recombinante) de la enzima deficiente en la vena del ni\u00f1o (por v\u00eda intravenosa) una vez a la semana.&nbsp;<\/p>\n\n\n\n<p>En pacientes con MPS I, la laronidasa (<a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2023\/125058s246lbl.pdf\">Aldurazyme<\/a>), a<strong> <\/strong>Este medicamento, aprobado por la FDA en 2003, se utiliza para reemplazar la enzima \u03b1-L-iduronidasa faltante. Para la MPS II, la idursulfasa (<a href=\"https:\/\/www.accessdata.fda.gov\/drugsatfda_docs\/label\/2013\/125151s0152lbl.pdf\">Elaprase<\/a>), a<strong> <\/strong>El medicamento, aprobado por la FDA en 2006, se utiliza para reemplazar la enzima iduronato-2-sulfatasa faltante.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Trasplante de c\u00e9lulas madre hematopoy\u00e9ticas (TCMH)<\/strong><\/li>\n<\/ul>\n\n\n\n<p>El TPH solo se utiliza para la MPS I, ya que es m\u00e1s eficaz en el tratamiento de la MPS I (s\u00edndrome de Hurler) si se inicia en etapas tempranas de la vida. En esta terapia, la m\u00e9dula \u00f3sea del paciente se reemplaza con m\u00e9dula \u00f3sea sana de un donante, que puede producir la enzima faltante.&nbsp;<\/p>\n\n\n\n<p>El TPH puede ayudar a preservar la funci\u00f3n cognitiva si se realiza antes de que se desarrollen s\u00edntomas importantes.&nbsp;<\/p>\n\n\n\n<p>Su proveedor de atenci\u00f3n m\u00e9dica tambi\u00e9n puede sugerir otros tratamientos de apoyo, como cirug\u00eda, otros medicamentos y fisioterapia, para controlar los s\u00edntomas o desaf\u00edos y mejorar la calidad de vida en pacientes con MPS I y II.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Esperanza de vida<\/strong><\/h2>\n\n\n\n<p>Los tratamientos actuales (p. ej., TRE y TPH) pueden mejorar la tasa de supervivencia si se inician antes de que se produzcan da\u00f1os irreversibles. Los pacientes con MPS I y la forma grave (s\u00edndrome de Hurler) tienen una esperanza de vida inferior a 10 a\u00f1os sin tratamiento. Sin embargo, con un tratamiento oportuno, las personas con casos leves pueden vivir hasta la edad adulta o alcanzar una edad avanzada.&nbsp;<\/p>\n\n\n\n<p>En la MPS II, el r\u00e1pido deterioro f\u00edsico y neurol\u00f3gico puede acortar la esperanza de vida a aproximadamente <strong>10\u201320 a\u00f1os<\/strong> Sin tratamiento.
